*Asynchronous therapy means you communicate with the therapist anytime day or night in the secure server, privately and confidentially between just you and the therapist. You share information in the server and the therapist reviews it and replies. This is like having a therapist in the digital world where you can leave a message and have a reply often the same day! Licensing laws in the US limit online therapy practice to client residency and therapist regulatory board location (still in 2019 licensing boards have not figured out the logistics of a national standard). For now, asynchronous services are offered to residents of DC and residents of Florida. Anyone in these locations can sign up for the asynchronous online therapy option because these jurisdictions are where LaRose is also licensed. The monthly fee is a subscription and it is charged upon signing up for the service in the server using your credit card (your ID and your credit card will be verified with online file sharing before you get started). Time and space is limited for the asynchronous option, not only to how many clients are enrolled but also to how much time LaRose can put into reading and replying. Complex mental health cases are NOT ideal for online based models and those who are under the care of an MD taking psychotropic medications may do better in streaming sessions and in office sessions. Not everyone who seeks asynchronous options will be an idea candidate. If you are unsure if the asynchronous therapy option is for you, please just give me a call on my cell and we’ll talk it over! #TalkifUwant!

* Clinical Hypnosis (CHT) sessions are scheduled after an initial Pre-CHT consultation session. Some of your questions about hypnosis may be answered in the initial consult however the pre-cht session is not completed during a free consult. Free consults are quite interactive and busy as are pre-cht sessions – it is not possible to complete both consults in one hour. The one hour educational session is optional, and recommended so that the CHT/RRT specialty can be viewed as the best option in your care (not everyone is the best candidate for CHT so the CHT consult will be helpful in determining the desired outcome. The practice now offers one-hour CHT sessions for the most rapid and brief intervention possible (and to reduce costs). Why are CHT sessions more expensive? The training involved is one reason, ongoing practice and implementation of the method is the other, it is a regulated practice in some jurisdictions such that it is a specialty area (not all providers can perform clinical hypnosis) and as a rapid resolution service with more immediate outcomes often little to no follow-up is needed (particularly with the 3.5 hour sessions). #TalkifUwant!

* Concierge services are unique, in that these are customized and facilitated to your specific request(s) and as such these services are highly targeted. They seldom include full assessment variables, they are provided more in a “life coaching” type format. In a clinical format there is little specific direction; the clinical model would not include advice, it would be based on comprehensive intake, assessment, diagnosis and treatment. Coaching is more client directed, in what is being targeted; it is not diagnostic based (which is what clinical counseling does) and coaching would include specific directions on how to succeed by a very structured plan. Concierge services and coaching services are often much more costly as the requirements of the professional are enhanced in the level of participation in life changes (where clinical indicators do not prevent such a service, which also can happen). If you are unsure about concierge, coaching and clinical options, please just give me a call on my cell and we’ll talk about it! #TalkifUwant!

What if I have Health Insurance or want to use EAP or I need Reduced Fees?
The practice of LaRose has expanded its digital and telehealth services (video, chat, online shared journaling, file sharing) and 24/7 asynchronous access options along with traditional in office and face to face sessions in both DC and Florida. The practice also offers onsite weekend service options for retreat get aways customized to you and your family needs.
After accepting as many as 18 – 20 carriers of insurance via major national brands and federal plans, including EAP of other program payers of varying kinds the delay of payment and non-payment behaviors of the insurance industry became overwhelmingly burdensome. When my practice, with a full time staff member and at one point a professionally trained insurance billing agent, hit the 2.87 hour mark of pursuing payment problems for each 1 hour of patient care – I moved to a gradual reduction of payers accepted, over a 5 year window to eventually become a self pay practice only. While there are many (if not most) providers in the US who take insurance payments 100% of them have departments and management issues with all of the 3rd party payors and the costs in managing “delay of payment behaviors” is prohibitive to effective care and effect focus on care. If you are insurance dependent, that is you must rely on health insurance to access care, and you do not have out of network benefits, you should contact your carrier to find a provider who takes your insurance. If you are able to pay out of pocket, what this practice recommends as the best way to be treated effectively, efficiently and with lower costs overall to healthcare and for your care, you should schedule with TalkifUwant.
So many services that are available with TalkfUwant are not covered by insurance anyway (and in some scenarios where coverage is available the allowed payment they make can be as low as $4.00). Hypnosis, onsite services, online services, asynchronous services – nearly all of these are denied by third party payors. And for those that do pay, the payors often reject the initial claims requiring providers to spend hours online and on the phone to justify a claim. Without the burden of insurance clients can access the secure/encrypted client portal 24 hours a day and use those services they wish to pay for without concern. With self scheduling, and an array of services online, as well as multiple non-traditional treatment options, clients have more control over their mental healthcare, and often use fewer services, visit when only when needed and return to happier living in shorter time frames compared to traditional fill the open spot approaches in scheduling. Clients with lifelong mental health conditions, and those who do not believe brief treatment or rapid resolution methods will be effective, and those who are dependent on health insurance for mental health services are still seen by the practice (as long as the insurance provides out of network coverage). If in the initial consultation it is determined that a lifelong mental health risk is a factor, this will be discussed with you either at that time, or at the second session when the treatment plan and treatment trajectory is discussed.
Reduced Fees. If during the course of your care a major life event occurs and the costs of care become prohibitive arrangements will be made to accommodate your treatment to either its successful termination (your treatment plan goals are met) or to your satisfactory linking to an alternative provider at some reduced fee. Reduced fee agreements require additional information which can and will be discussed at the time the life event is realized. If during a major life event you have health insurance and you need reduced costs for care, you will be asked to contact your carrier for a list of providers who take your insurance, if / when a life event is known.

Published Fees & Ordering Services
The published rates are based upon either an hourly rate or a fee for service. Most fee for service rates assume 8 hour days. All fees must be paid in advance of the service, unless other payment arrangements have been made prior to the services being rendered. Rates can change from time to time, however all published rates will be honored unless you are notified in writing. All prices and fees for a wide array of services are listed on this page; not all services will apply to you and your particular situation. Concierge services are all prepaid and packaged; those that are not used within the time allot are NOT carried over—these are “use it or lose it” options. Concierge services are an emerging practice, born mostly out of the frustrating limitations that insurance companies place on providers—in helping clients get the kinds of services they need and want (and these are ideal when mental illness diagnosis is not a factor or when a sole emphasis on health and getting better is the preferred intervention). More about how concierge services work—can be reviewed here; this can be selected as a reason for service when scheduling online (24 hours a day) and a customized service will be discussed and developed, if this option is the best fit to your particular situation. Concierge packages are time limited, prepaid, and must be used in the time frame specified or any unused session times are forfeited. Plus travel and plus pass thru means that in addition to the hourly fee, concierge services may included added costs; costs for meals for example are passed thru to the client at cost. Cost for travel is at the current per mile allowance and/or at the cost of the travel mechanism utilized to meet at a location (cab, uber, airfare, hotel, as applicable to your concierge option). Open access means users purchase a full year of traditional in the office sessions, at a reduced rate for the year, using up to the number allotted, within a 12 month period. Open access services are paid in advance, and like concierge, are time limited; unused sessions are forfeited at the end of the year. All services may be ordered online, if placing an order now is more convenient.
Formal Instrumentation Fees
Instruments are often included in written assessments, to support or rule out clinical findings. When assessment instruments are indicated by the therapist, or requested by the client—and agreed upon (for pre and post treatment evaluation purposes, in some expert opinion scenarios, or in cases of more serious and long term treatment) the cost for the Psychometrically Evaluated instrument varies according to rates that are set by the developer and/or publisher, with additional hourly fees and processing fees (as applicable to your particular situation).
Assessment, Diagnosis, Treatment Plan, Termination Goal
Each client who is seen in private practice will be assessed, with an optional written diagnostic impression (and appropriate rule out considerations). Formal assessments and instrumentation are more costly, and not all clients request or require psychometric diagnostic documentation. Formal written clinical assessments, if requested or if clinically indicated will be discussed in some detail during the initial visit. Treatment options and options for care will vary if and when a interfering mental disorder appears to be at play (meaning, if life coaching and concierge services are sought yet mental health variables limit this approach a clinical diagnostic model may be necessary).
In the event there is not a diagnosis (or there are other considerations) a formal written assessment could include the basis of such a determination. Suggested interventions and various treatment options will be discussed at the second or third session, for client consideration and approval. Health insurance may or may not be applicable, depending on network status and non-covered services.
Assessments (formal written psychometrics or clinical picture impressions), treatment plans (as agreed upon before treatment begins) and a termination timeline will be addressed during the intake session at the office (or via the online video/chat sessions) of/with LaRose.
HAVING TROUBLES WITH LINKS ON THE TALKIFUWANT WEBSITE?
Please forgive some of the non-working URLs. TalkifUwant.com is undergoing a website rebuild, transfer, platform transition, as well as being hosted with a new hosting company. With that change, there are “growing pains” (perfect for a behavioral health platform!). From the early 2000’s know that we’re working to update and successfully transition well over 100 pages in the domain – with material that dates back to the late 1990’s! And for a sole provider / practitioner — it’s allot! And while I’m here, here’s a shout out to the folks at brighter vision who have been working with TalkifUwant since October 2018, to make this very large transition – a success!
